How about costs and insurance?
Our fee for a bariatric psychological evaluation including the testing, interview and report is $550.
We have an insurance billing department to check your coverage for you. Our billing folks will explain any co-pay or deductible due at the time of the evaluation and file your claim for you.
You only pay the co-pay or deductible at the time of the evaluation. If your insurance fails to pay, you will be responsible for your charges.
